Position DescriptionEmployees in this classification provide patient care management to an assigned clinic in the areas of clerical and clinical duties. In clinical setting, the employee receives direct supervision from physician or nursing supervisor in either oral or written form. In office setting, the employee receives direct supervision from department administrator. Work is performed under usual office and clinic conditions. May be stressful when working under emergency conditions.
Major/Essential Functions- Communicates, reports, and documents accurately, honestly, and in good faith.
- Create Reports data.
- Identifies and reports alterations in patient responses to assist in identifying problems and formulation of goals/ outcomes and patient-centered care plans in collaboration with patients, their families, and the interdisciplinary health care team.
- Monitors nursing processes to determine patients' and their families' physical and mental health status, needs, and preferences, based upon interpretation of comprehensive health assessment findings.
- Monitors communication of patient data using Electronic Health Record to support decision making for the delivery of optimal patient care.
- Works with faculty physicians to develop, implement, and evaluate patient and family teaching plans to address health promotion, maintenance, and restoration.
- Participates in emergency preparedness drills in the department.
- Inspects examination/treatment rooms to ensure they are adequately stocked and compliant with infection prevention and control policies.
- Prepares and operates necessary equipment and instruments for examinations and treatments.
- Complies with the identification of patient needs, implements, and facilitates continuity of care
- Evaluate and report patient outcomes and responses to therapeutic interventions.
- Complete proper follow-up on patient care issues/messaging.
- Disinfects medical equipment and instruments.
- Calls patients back from the lobby to the exam room.
- Takes vital signs and measurements, then accurately documents in the patient EMR.
- Creates patient intake forms.
- Reconciles patient medications and updates medication list as needed.
- Receives all patient telephonic communication.
- Monitors and responds to all communication. Following ambulatory clinic policy regarding documentation, Weekly reviews medical supplies to ensure they are adequately stocked.
- Ensures pharmacy and patient refill requests, paperwork, and concerns are resolved within 24 to 48 hours of receipt.
- Monitors schedules, room utilization and clinic patient flow to ensure maximum efficiency.
- Ensures physician orders for tests, referrals, and/or lab tests are in the patient's EMR.
- Reviews the daily reports to determine if results are critical or normal.
- Contacts patients to advise of normal results via EMR Patient Portal or telephone (whichever applies).
- Contacts the ordering physician to advise of critical results.
- Ensures all clinical staff and providers meet all Joint Commission standards.
- Conducts regular quality audits to ensure proper care and documentation is complete.
- Completes Occurrence Report via online UMC reporting website.
- Completes Annual Nursing Core Competency Training and Skills Fair.
Required QualificationsEducation:
- A minimum of a High School diploma or equivalent.
License/Certification:
- Graduation from an approved certified program for assistants in the area of required specialty.
- Certification by an approved accredited certifying agency in the specialty that particular work will be performed.
